Once you have installed the app, you are able to create Fabulous Custom Fields for your Jira instance.

Configuration

Basically, a configuration of Fabulous Custom Fields is the same as for pre-defined Jira Custom fields.

Go to: Settings → Issue → Custom Fields → Create Custom Fields

...

As our fields are based on the new field types you can create as many fields as you need, add specific names for them, and configurations that represent your needs. For example, you can create several fields of the type ‘User Notification' and naming and configure them specifically for the project’s needs.

Move to the Advanced section → start typing field type name → select field from the list and click Next

...

Add the corresponding field name and description (this name will be displayed in your issues view) and click Create

...

Associate your field with the appropriate screens, and click Update

...

Now, you can find your new field in the Custom fields list. Also, you can configure the field according to your needs, just select 'Contexts and default value' in the field menu

...

Click 'Edit custom field config' and proceed with the direct field configuration, additional information you can find in the Using Fabulous Custom Fields section.

...

That all, simple and usually like general Jira fields, now you can get experience with the field in the corresponding issue screen view.
